The reason for which the applicant has made an application. If the place/property belongs to the applicant, the property card (7/12) should be attested by the concern office. *
If the place/location given by the applicant does not belong to the applicant, a lease agreement should be taken from the owner of the place on stamp paper and should be mentioned in the report. *
If the place/location is in an Apartment/Society, NOC of the society or the apartment should be taken to start the business and should be mentioned in the report.
If the place/location is in the jurisdiction of Grampanchyat, Municipal Council, Municipal Corporation, or Cantonment Board, legal NOC should be taken from the concern office and should be mentioned in the report. *
Is there any Temple/church/mosque/court/Government office, School/college/Hospital near 100 meters of the place/location of the Shop/Bar? If yes, how far is it from the entry of the Bar/Shop? It should be measured in meters and feet and should be mentioned in the report. *
Regarding the people staying near the place/location, is there any problem for them or any law and order problem that will arise if permission is granted? Written NOC should be taken from 7 to 8 people and should be mentioned in the report. *
Is there any State/National Highway passing from 75/50 meters from the place/location and nearby 220 meters where the population should not be more than 3000? If yes, the letter should be taken from the concern department and should be mentioned in the report. *
Completion plan/certificate should be taken from the concern Grampanchyat, Municipal Council, Municipal Corporation, or Cantonment Board of the built-up area. It should be mentioned in the report. *
Is there any offense registered against the applicant? If yes, the details should be mentioned in the report. *
Is there any law and order problem that arose earlier or will arise in the future at the place/location? The details should be mentioned in the report. *

What is the parking position near the place/location? *
